Principles of effective use of non-steroidal anti-inflammatory drugs

The article provides an overview of references on the rational use nonsteroidal anti-inflammatory drugs (NSAIDs) in modern medicine. Nonsteroidal are a group with different chemical structures (mostly acid derivatives) that have anti-inflammatory, analgesic, antipyretic, antiplatelet (acetylsalicylic acid, acetylsalicylate, ketoprofen, diclofenac sodium, niflumic indomethacin) effects. NSAIDs divided according to selectivity action relative cyclooxygenase (COX) isoforms: non-selective COX inhibitors, selective COX-1 approximately equal inhibition and COX-2, COX-2 inhibitors. They characterized by general pharmacological properties: high degree absorption gastrointestinal tract; binding albumins; same volume distribution; ability accumulate focus inflammation. Indications for are: acute arthritis chronic arthritis; pain syndrome various nature (lower back syndrome, joint soft tissue injuries, migraine, dysmenorrhea, preoperative postoperative pain, renal colic, fever rheumatic non-rheumatic diseases). Аdditional indications prescribing pleurisy, pericarditis, erythema nodosum, polycystic lung disease, sciatica. most frequent dangerous side effects include gastrointestinal, kidney complications. Special attention is paid cardiovascular safety and, above all, inhibitors because risk events. effective drug best tolerability should be selected specific patient. Before starting NSAID therapy, patient's age, comorbidities, previous medical or surgical history, concomitant medications (including agents, anticoagulants, corticosteroids, ACE serotonin reuptake inhibitors), H. pylori infection, blood pressure monitoring considered.
